How to use this Student Information Sheet:

This editable student survey can be used as a back-to-school activity on the first day of school. Promote equity, growth mindset, and class community in any subject area using the sample question prompts provided with this resource.

The Student Information Sheet is designed to go beyond the typical “get to know you” questions and instead foster meaningful connection and inclusion from day one. Whether you’re teaching elementary students or secondary learners, this resource offers a structured yet flexible format for understanding each student’s background, goals, and needs. The Student Information Sheet is ideal for building relationships in classrooms where trust and understanding are essential to student success. By encouraging students to reflect and share, you create a space where every learner feels seen, heard, and supported.

These get to know your student questions are broken up into four sections:
The first section will prompt the student to provide information about their identity.
The second section will prompt the student to share about their family and cultural community.
The third section will inquire about interests and responsibilities.
The final section will address health-related concerns, and offer the student the opportunity to share any additional information.

The layout of the Student Information Sheet has been thoughtfully organized to help educators gain a full picture of each learner while maintaining a respectful tone. By addressing identity, culture, interests, and well-being, this form encourages students to reflect on what makes them unique and important. These categories provide valuable insight into how students experience the world around them, as well as how they may interact with classroom material. Teachers can use the responses on the Student Information Sheet to inform lesson planning, group assignments, and even classroom routines, ensuring that instruction is responsive and inclusive.

To promote growth mindset, this student information sheet also prompts students to consider their goals and anticipated challenges for the school year.

I guess this is suitable for any secondary classroom since reading is always a requirement. The printable is geared towards English students because it asks specific questions about reading and native languages. I guess this is suitable for any secondary classroom since reading is always a requirement. This download includes an editable powerpoint template to change the questions though, so I will do this for my history classes and maybe ask a question about their own family histories or something related to subject content.
